Transition Workers
How can I get help from a Transition Worker?
 
Contact your local adult social care office to ask to speak with the local Transition Worker about a question and/or make a transition referral.  A referral is letting the council know you would like support and what help you think would help you.

Information for the Homeless
We offer free, impartial advice and information about homelessness and preventing homelessness. We aim to help people to keep their home or help them to find one that meets their needs. We can:
 
Help you to understand the different options that are available in Carlisle.
Prevent a homelessness situation from occurring, wherever possible.
Explain the processes of homelessness prevention assist in locating alternative accommodation, such as a housing association, local authority or privately rented property.
Provide you with specialist information and advice on your available options.
Provide online information and advice relating to housing and homelessness and focused on meeting specific needs.
Home from Home Care
Care homes for adults with special needs, learning disabilities, autism, epilepsy, cerebral palsy, PMLD, complex health and mental health.
 
Youth Offending Service
Cumbria Youth Offending Service is one of 155 Youth Offending Teams in England and Wales. At a national level, the Youth Justice Board has a statutory duty to monitor the effectiveness of the youth justice system and give advice to ministers. In order to fulfil its duty, the Board sets performance targets for Youth Offending Services and monitors their work. The Board provides additional financial support to Yot partnerships in the shape of ring-fenced grants to promote good practice and/or to develop new services.
 
Inspira
Inspira provides careers information advice and guidance for young people with an EHCP from year 9 upwards. We work closely with schools and colleges to support transition planning, the help we provide is bespoke and tailored to meet individual needs and we welcome the involvement of parents. An Inspira Adviser would meet with the young person in school in advance of their EHCP review, in meetings, our role is to advocate on behalf of the young person and family. Inspira works with a range of providers and agencies to ensure that we can support young people and their parents/carers to prepare for adulthood.

Housing - Barrow Borough Council
The Housing Service is part of the Cumbria Choice, Choice-Based Lettings Scheme. Cumbria Choice is where all the main social housing providers in Cumbria now advertise their vacant properties. 
 
To apply for properties you first have to register on the Cumbria Choice Scheme and complete an online application form - register now.  From here you will be able to get information on how the scheme works and view frequently asked questions.
 
Cumbria SEND IASS
Cumbria SEND IAS Service offers confidential, impartial information, advice and support to children and young people (0-25 years old) with special educational needs and/or disabilities and their parents and carers.  The support is offered so they can make informed decisions about their education or that of their child or young person. Our service includes signposting to health and social care advisory services, as well as other agencies, where appropriate.

What is disability related expenditure (DRE)?
 
Disability related expenditure is money that you spend on items and services that help you manage or cope with a disability or a long-term health condition.  It is money that we may recognise as an 'outgoing' when we do a financial assessment to find out how much you can afford to pay towards your care.

What is Healthlands?
 
Heathlands is a community led by our members to promote and enable the personal development and wellbeing of  our members.
 
We aim to help individuals to make positive decisions about their lives and overcome any issues that may hold them back. This is achieved by supporting them to engage in or access a range of activities and opportunities. These could be educational, training and moving towards employment, personal development or social. The aim is to have an impact on our members personal lives, improving such areas as self-esteem, confidence or health and wellbeing. By working with a diverse range of groups, partners and communities one of the project's aims is to promote social inclusion and create new opportunities.

Parent Carers
Parent Carers care for their children who are ill or have a disability.  Parent Carers are most likely to be caring the longest. The responsibilities may be even greater in a situation where the Carer is a sole parent, who has more than one person to care for. 
 
We provide the same high level of support for Parent Carers as we do to all our Carers, including:
  • Support that is; face to face, peer, group and drop-in, telephone & online based support.
  • Information and advice on caring
  • Advice and support on contingency planning
  • Referral and signposting to support services; coordinated joint-working
  • Peer support groups, befriending, Carers cafés & activities
  • Employment & rights, employability & support
  • Emotional support, planning ahead and bereavement support

Cumbria Youth Alliance - CYA
We provide direct support in a variety of settings including online, schools, alternative provision, youth, and community settings. We have several new projects and continue to deliver on our existing projects across Cumbria, ensuring that our projects continue to meet the needs of young people and enable them to reach their full potential. Post-16 Officers
We undertake assessment, planning and review activities for Post 16 Education Health and Care Plans. Liaising with colleagues across schools, FE colleges and residential specialist colleges. Providing advice to team members and other professionals including social care and health on education aspects of SEND and transition.
